- 5:00 PM (Evening Session)

Document Submission Deadlines
Documents may be submitted anytime before June 15th:
- Physical Exam Results
- TB Test Results (blood test, skin test, or chest x-ray if applicable)
- COVID Vaccine Card
- Completed Livescan Form (paper copy required)
- 283B Form (paper copy required)

Uniform Fitting & Supply Pick-Up
- Students doing clinicals at Milpitas/San Jose: Try on uniforms & receive your supplies TOMORROW!
- Students doing clinicals Sacramento: Supplies & BLS-CPR class on June 8th(location to be announced).
- Students doing clinicals Santa Cruz: Tentatively scheduled for June 14th(exact time TBA).
