We’re excited to have you join our Home Health Aide (HHA) class! Please follow these steps to secure your spot and complete your enrollment:
✅ Secure Your Spot
Submit a $50 deposit (only 15 students can enroll—first come, first served). After the payment, please send the screenshot of receipt to 408-256-0763.
✅ Submit Your CNA Certificate/License
Send a copy to 408-256-0763.
✅ Prepare Your Live Scan for HHA
You’ll need 3 copies of your completed Live Scan form. Use these forms:
- 📌 HHA Live Scan Form
- 📌 Live Scan Sample
- 📌 Same as CNA form. OCA is your SSN.
✅ Bring Required Materials
- 📌 1 large yellow envelope
- 📌 3 postage stamps
